Enrollment Checklist

Steps

1. Visit https://girardcity-oh.finalforms.com/ to begin the registration process for your incoming kindergarten student through Final Forms.

​

2. Once you have completed the FinalForms registration, click here or scan the QR code to schedule a time to bring in the required documentation and residency forms.

​

Required Documents 

• Child’s Birth Certificate - An original (or certified copy) of the student's Birth Certificate must be presented at registration.

• Child’s Social Security Card - An original Social Security Card must be presented at registration.

• Parent/Guardian Ohio Driver's License or State Issued ID Card - Must be presented at registration appointment for proof of identity.

• Student's Immunization Records - Along with documentation from the physician.

 

​Proof of Residency (One of two options)

1. Homeowner/Renter

• Affidavit of Current Residency Res 1 (Must be Notarized)

• Affidavit of Prior Residency Res 2 (Must be Notarized)

• Mortgage or bank payment book with name and address, or lease agreement with name and address

• Two pieces of business mail with the Girard address

 

2. Living with someone else

Parent/Guardian must provide

• Affidavit of Current Residency Res 1 (Must be Notarized)

• Affidavit of Prior Residency Res 2 (Must be Notarized)

• Affidavit of Residency Res 3b (Must be Notarized)

• Two pieces of business mail with the Girard address

Homeowner/Renter must provide

• Affidavit of Resident Homeowner Res 3a (Must be Notarized)

• Mortgage or bank payment book with name and address, or lease agreement with name and address

• Two pieces of business mail with the Girard address

 

Special Situations

• Custody Decree - (if applicable) Complete, current legal documentation must be presented at the time of registration. Notify the registrar of any future revisions.

• Guardianship - (if applicable) Complete, current legal court documents must be presented at the time of registration.

• Medical Plans - (if applicable) Diabetes, Seizures, Asthma, etc.

An official request will be made to the previous school for these documents as appropriate; however, having the documents upfront facilitates a smoother transition.

• Special Education Paperwork - (if applicable) The current ETR/MFE and IEP for students with disabilities, or the current Section 504 Accommodation Plan information must be presented at the time of registration.

• RIMP - (if applicable) The current Reading Improvement Plan. 

• Gifted Services - (if applicable) The current WEP for students with gifted identifications. 

• English Language Learners - (if applicable) The current LEP for English Language Learners.
